CMAS: a rich media annotation system for medical imaging

I saw the funniest presentation ever at the SPIE Medical Imaging conference last week in San Diego. Few researchers from HP were showing a new method for annotating medical images. I wish they had talked to a radiologist before presenting the paper and making a fool of them selves. You have see it to understand what I mean. Now that you have seen the video, here is what want a radiologist to do:

  1. To annotate an image first the radiologist identifies an image with abnormality.
  2. Then he has to become a weather man and stand in front of a blue screen and a video camera.
  3. Now he has to capture video of himself pointing to the virtual abnormality on the image.
  4. Then some how save this video - which is not DICOM
  5. Then send to a clinician - again not using the RIS or HIS but some other way.
  6. Lastly get fired because you wasted whole day annotating a single image and rest of the work was delayed.

On average a radiologist spends 90 seconds reading a radiograph and about 7 minutes interpreting a CT study. What ever annotation methods engineers and programmers develop has to be part of the 90 seconds for a radiograph and 7 minutes for a CT examination.

Advice for Engineers and Software developers: Please! Please! Consult the users (radiologist) before designing a system.

Future of IDXRad

Well before we talk about future of IDXRad, let's go over some history.
IDXrad was originally called DECrad and was transferred to IDX several years ago who changed the name to IDXrad. DECrad specifications were developed primarily (if my history is correct) by Ron Arenson when he was at Penn and by RISC.
RISC, Radiology Information Systems Consortium was started, in the late 80s and early 90s, to promote radiology information systems when few were available. DECrad (now IDXrad) was an outgrowth of RISC's efforts. An independent users group was formed from users of DECrad under RISC. This group has been active ever since as the DECrad Users' Group and now as IRISS, the IDXrad Radiology Information System Society.
In the meantime SCAR, Society for Computer Applications in Radiology was formed as an individual member organization within RISC. SCAR and RISC merged into one membership organization under the SCAR name in 1996. IRISS remains an independent users' group with SCAR, just as formerly with RISC.
From what I understand (and please correct me if I am wrong and misinformed), the initial deal with IDX specified that as long as IDX supported the IDXRad product they could sell it to users and when ever they stop supporting the product, the IDXRad code is to be returned to the RISC (now SCAR) who will then make it an open source project.
If the above is true, what happens now that the IDX has been bought by GE? What I hear is that IDXRad v 9.x (byproduct of DECrad) will no longer be supported by GE and the IDXRad v 10.x will be the new GE RIS. If that is the case will SCAR ask for the source code of IDXRad and release it? These all sound like rumors to me. If anyone knows the real deal please comment.

Poetic technologies offers futuristic workstation solutions for PACS systems. Their workstations are gigantic and take up entire room. These look extremely cool and are very photogenic. But are not practical at all. The biggest problem is the fixed chair which has similar mobility as a car seat. Although very comfortable for a short while, but if you sit on it longer then it gets very tiring as one does after driving for a while.

Radiologists work from 8am to 5pm non-stop and the gluteal region gets very tired of sitting in one position. We have seen in our research that comfortable chairs with wheels provide the least fatigue.

Most academic institutions usually have trainees that observe or actively participate in the interpretation process and the configuration of this workstation limits collaborative interpretation. Only one person can sit and others have to stand, which becomes impractical for use of the workstation in an academic environment. Similarly, centers where clinicians frequently consult with radiology in the reading room, will find the workstation causing hindrance in collaborative consultation.

Hey if you are a geeky radiologist who wants to have a futuristic look of his/her office, then this baby is for you. Would look great at home to show off.
