CMAS: a rich media annotation system for medical imaging

I saw the funniest presentation ever at the SPIE Medical Imaging conference last week in San Diego. Few researchers from HP were showing a new method for annotating medical images. I wish they had talked to a radiologist before presenting the paper and making a fool of them selves. You have see it to understand what I mean. Now that you have seen the video, here is what want a radiologist to do:

1. To annotate an image first the radiologist identifies an image with abnormality.
2. Then he has to become a weather man and stand in front of a blue screen and a video camera.
3. Now he has to capture video of himself pointing to the virtual abnormality on the image.
4. Then some how save this video - which is not DICOM
5. Then send to a clinician - again not using the RIS or HIS but some other way.
6. Lastly get fired because you wasted whole day annotating a single image and rest of the work was delayed.

On average a radiologist spends 90 seconds reading a radiograph and about 7 minutes interpreting a CT study. What ever annotation methods engineers and programmers develop has to be part of the 90 seconds for a radiograph and 7 minutes for a CT examination.

Advice for Engineers and Software developers: Please! Please! Consult the users (radiologist) before designing a system.

Future of IDXRad

Well before we talk about future of IDXRad, let's go over some history.

IDXrad was originally called DECrad and was transferred to IDX several years ago who changed the name to IDXrad. DECrad specifications were developed primarily (if my history is correct) by Ron Arenson when he was at Penn and by RISC.

RISC, Radiology Information Systems Consortium was started, in the late 80s and early 90s, to promote radiology information systems when few were available. DECrad (now IDXrad) was an outgrowth of RISC's efforts. An independent users group was formed from users of DECrad under RISC. This group has been active ever since as the DECrad Users' Group and now as IRISS, the IDXrad Radiology Information System Society.

In the meantime SCAR, Society for Computer Applications in Radiology was formed as an individual member organization within RISC. SCAR and RISC merged into one membership organization under the SCAR name in 1996. IRISS remains an independent users' group with SCAR, just as formerly with RISC.

From what I understand (and please correct me if I am wrong and misinformed), the initial deal with IDX specified that as long as IDX supported the IDXRad product they could sell it to users and when ever they stop supporting the product, the IDXRad code is to be returned to the RISC (now SCAR) who will then make it an open source project.

If the above is true, what happens now that the IDX has been bought by GE? What I hear is that IDXRad v 9.x (byproduct of DECrad) will no longer be supported by GE and the IDXRad v 10.x will be the new GE RIS. If that is the case will SCAR ask for the source code of IDXRad and release it? These all sound like rumors to me. If anyone knows the real deal please comment.

But are not practical at all. The biggest problem is the fixed chair which has similar mobility as a car seat. Although very comfortable for a short while, but if you sit on it longer then it gets very tiring as one does after driving for a while.

Radiologists work from 8am to 5pm non-stop and the gluteal region gets very tired of sitting in one position. We have seen in our research that comfortable chairs with wheels provide the least fatigue.

Most academic institutions usually have trainees that observe or actively participate in the interpretation process and the configuration of this workstation limits collaborative interpretation. Only one person can sit and others have to stand, which becomes impractical for use of the workstation in an academic environment. Similarly, centers where clinicians frequently consult with radiology in the reading room, will find the workstation causing hindrance in collaborative consultation.

Hey if you are a geeky radiologist who wants to have a futuristic look of his/her office, then this baby is for you. Would look great at home to show off.

Vocada Awarded Patent for Merging Voice Messaging Data First Used in Failsafe Medical Communications System

Vocada is a great application - it solves a lot of communication problems that currently exist in reporting results to clinicians to get care at the point of delivery.

Here is how currently radiologists report significant findings to clinicians about a radiology exam:
1. First try to find who ordered the study
2. Find their phone number or pager
3. Call or page
4. Usually clinician not available a the phone number called and usually clinician with a patient and is unable to answer the page.
5. Radiologists wait for clinician to return call and usually forget about the case and the important finding is not reported immediately. Report does get deliver by mail or fax but is not seen up to 24 to 48 hours later.
6. And radiologists never know if the clinician actually read the report.

This above workflow creates a lot of errors in management and delay in treatment.

In the world of Vocada here is what happens:
1. Radiologists finds an abnormality that needs to be reported and dictates it.
2. Clicks on the Vocada icon.
3. Vocada pages the clinician or calls.
4. Automatically plays back an audio file of the findings to the clinicians.
5. Let's the radiologist know when a clinician got the results.
6. If clinician does not answer - radiologist is notified of the delay.

This workflow so much time. In the old workflow, some radiology practices have time blocked off during the day when radiologists call to report findings. Vocada not only improves communication, but improves radiologist's productivity and timely patient care.

Last Fall, Stanford University began a partnership with Apple to publish and host lectures for download via the iTunes Store. Called Stanford on iTunes, it's been a resounding success and now Apple is looking to replicate th! at success with other schools around the country and world.

iTunes U. (for University) will be a partnership between Apple and schools for hosting and distributing audio and video lectures, podcasts, and vidcasts to their student bodies. Modeled after the Stanford on iTunes program, iTunes U. will be a free service and allow a school to create an environment for instructors to upload their audio and video podcasts for distribution to their student bodies.

It will likely incorporate some of what Apple has been calling Quicktime 2 RSS, a set of tools for recording a lecture and then encoding it for playback on a computer or an iPod.

iTunes U. is looking like a powerful way to expand how students get content. We're living in an increasingly digital world, we may as well embrace learning digitally, and it's good to see A! pple take the lead in providing the tools, the hosting, and th! e techno logy to allow such a shift to happen. I'm definitely going to be lobbying for my employer (a university) to get involved in this program.

I've been complaining that Apple hasn't been as competitive as they should be in the Education market for some time. Perhaps this is a sign that the times are a-changing. Truly interesting.

There is so much misinformation about HIPAA (Health Insurance Portability and Accountability Act) out there it is ridiculous. From someone who has written book chapters about HIPAA and been invited to speak at major radiology meetings on this topic, I end up just clarifying myths about HIPAA in my talks.

It is amazing how consultants and HIPAA officers in institutions put fear HIPAA officers in institutions put fearin their institution and end up hindering patient care. HIPAA actually intended to help in information sharing, but because of misinterpretation of the law, institutional HIPAA policies end up being very restrictive.

I agree with most of the responses to Peter Frishauf’s commentary on Medscape. We need to do a better job educating health care professionals and decision makers in health care.

This is the latest PACS book out.  Second edition of a previous book with the same title.  Comprehensive, but does not cover all the topics.  There are some editorial mistakes.  Disclosure: I am one of the contributing authors for in this book.

RSNA PodCast? Good idea or an idiot's fantasy?

Wouldn't it be great if the RSNA annual meeting was PodCasted? Just imaging, you could take your media player or laptop and actually see talks and presentations that you missed because you were at another session which turned out to be horrible.

You could also see these podcasts at home if you were unable attend the meeting. Do you think RSNA will ever do this? Will some of the IT industry be interested in this - for example Apple, Google, Microsoft, GE etc? It could be a huge marketing oppertunity. 60,000 attendies - all rich radiologists or executives buying portable media player to view the PodCasts. Imaging RSNA branded video iPOD or maybe GE branded - hmm... maybe not.

Maybe one day they will do live PodCast of the entire meeting. I think I am going to propose this for this year or the next RSNA meeting. Let's see what reaction I get. Will post feedback here, when I get it.

How is this different from storing images on any USD media device or just USB flash memory? All they are doing is using it as a USB hard drive - a very expensive hard drive. So why all this hype?

Study shows radiology not safest place to be in hospital

This article gives a very bleak picture of radiology departments. A lot of errors in radiology occur because of misinformation or lack of information provided by clinicians ordering tests. Multiple studies have shown overuse and abuse or radiology service utilization by clinicians. We routinely see studies ordered that are not clinically indicated and try our best effort to educate the clinicians. To circumvent delay in getting a study done, clinicians frequently give incorrect clinical information to get a examination approved and done. This misinformation results in a lot of complications that we see in radiology departments.

For example, a clinician not notifying radiology about a patient with history of claustrophobia who is scheduled for MRI scan. The MRI scanner tube is narrow and a lot of patients feel claustrophobic in it. These patients then try to get of the scanner tube while the scan is going on and can hurt themselves.
